Job summary

AdventHealth in Orlando seeks an experienced RN for Care Management to coordinate discharge planning and ensure timely transitions across the care continuum.

You will lead multidisciplinary rounds, assess patient and family needs, and educate on post-acute services while aligning with CMS requirements and diverse populations.

The full-time role offers growth and collaboration within a faith-based health system and a supportive team environment.

Qualifications

• RN license required
• 2+ medical/hospital nursing experience
• Care Management/Utilization Management experience preferred
• Bachelor’s degree preferred for leadership roles or advancement

Responsibilities

• Actively participates in multidisciplinary rounds to review patient status, discharge plans, and progression.
• Educates patients and families on emotional, social, and financial aspects of illness and resources for post-acute care.
• Assesses readmissions and analyzes reasons to improve transitions of care.
• Organizes care conferences with the multidisciplinary team and communicates with Payors for post-acute authorizations.
• Reviews medical records and coordinates discharge planning including post-acute care arrangements.
• Develops discharge plans with contingency planning and ensures continuity of care from hospital to home or facility.
• Uses technology and standard work to ensure complete medical records and discharge reconciliation.
