Job summary

Qualifications

• Bachelor's Degree in Nursing or Social Work preferred.
• Active RN license required; LVN/LPN considered.
• Minimum two years of clinical nursing experience preferred.
• Knowledge of Medicare, Medi-Cal, and Medicaid programs and benefits.
• PCC experience preferred.

Responsibilities

• Coordinate and oversee resident care services during weekend operations.
• Participate in morning stand-up meetings to facilitate interdisciplinary communication.
• Explain care plans and treatment choices to residents and families.
• Monitor resident status and adjust care interventions as needed.
• Ensure timely and accurate clinical documentation and charting.
• Oversee admissions and discharges to ensure smooth transitions of care.
