Qualifications

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of practice.
• Experience supervising Physical Therapy Assistants.
• Experience with long-term care and/or geriatric care preferred.
• Well versed with documentation writing and record keeping.

Responsibilities

• Assess referred patients and provide direct PT treatment in accordance with physician orders.
• Develop and administer individualized treatment plans with clear goals.
• Set realistic, measurable goals considering patient capacity and tolerance.
• Document patient condition and progress; educate patients and families in therapy methods.
• Collaborate with all disciplines to provide patient-centered care and professional service.
