Infinite Medical P.C. in Holyoke, MA, is seeking a part-time Primary Care NP/PA to provide on-site care in long-term care facilities on weekends. The role offers a competitive salary between $60,000 and $90,000 per year and includes benefits such as paid time off and malpractice insurance.

The ideal candidate will hold an active NP/PA license with MA licensure in good standing, plus at least 6 months of LTC experience, strong communication skills, and a patient-centered approach to elder care.

Qualifications

• Active and unrestricted NP/PA/Physician license.
• Massachusetts license in good standing.
• Minimum 6 months of experience in long-term care or skilled nursing facilities.

Responsibilities

• Provide primary care services to residents in skilled nursing and long-term care facilities.
• Perform physical exams and medical history assessments.
• Diagnose and treat chronic and acute health conditions.
• Order and interpret diagnostic tests and lab results.
• Develop and adjust individualized care plans.
• Prescribe medications and monitor treatment effectiveness.
• Collaborate with facility staff and interdisciplinary teams.
• Educate patients and families about treatment options and care goals.
• Ensure a safe, respectful, and supportive environment for residents.
