Infinite Medical P.C. is seeking a Primary Care Nurse Practitioner/Physician’s Assistant to provide on-site care for residents in long-term care facilities in Holyoke, MA. This part-time weekend role emphasizes building meaningful patient relationships within a collaborative care environment.

The ideal candidate holds an active MA license (NP/PA/MD) with at least 6 months in long-term care, strong communication, and a passion for elder care.

Formación

• Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license.
• State-specific license in good standing in Massachusetts.
• Minimum 6 months of experience in long-term care or a skilled nursing facility.
• Strong communication, clinical, and critical thinking skills.
• Passion for elder care and person-centered healthcare.
• Prolonged standing/walking; occasional lifting up to 20 lbs; computer work.
• Competitive salary: $60,000 – $90,000 per year.

Responsabilidades

• Provide primary care services to residents in skilled nursing and long-term care facilities.
• Perform physical exams and medical history assessments.
• Diagnose and treat chronic and acute health conditions.
• Order and interpret diagnostic tests and lab results.

• Prescribe medications and monitor treatment effectiveness.
