Job summary

Infinite Medical Group is currently hiring a Primary Care NP/PA to provide on-site care to residents in long-term care settings. This part-time weekend position values meaningful patient relationships and a collaborative care environment.

The ideal candidate holds an active NP/PA license, MA license in good standing, and has at least 6 months of LTC experience. You will work with facility staff to deliver high-quality elder care and coordinate care plans with the interdisciplinary team.

Qualifications

• Active and unrestricted NP/PA/Physician license.
• Massachusetts license in good standing.
• At least 6 months of LTC/SNF experience.
• Strong communication and critical thinking skills.
• Ability to stand for long periods and assist moving patients.

Responsibilities

• Provide primary care services to residents in skilled nursing and long-term care facilities.
• Perform physical exams and medical history assessments.
• Diagnose and treat chronic and acute health conditions.
• Order and interpret diagnostic tests and lab results.
• Develop and adjust individualized care plans.
• Prescribe medications and monitor treatment effectiveness.
• Collaborate with facility staff and interdisciplinary teams.
• Educate patients and families about treatment options and care goals.
• Ensure a safe, respectful, and supportive environment for residents.
