Job summary

The Gallatin Center for Rehabilitation and Healing is seeking a Physical Therapist (PT) for PRN weekend staffing. The PT will assess referred patients, develop individualized treatment plans, and educate patients and families while documenting progress.

The role requires collaboration with an interdisciplinary team and adherence to HIPAA and patient rights. Qualified candidates have an accredited PT degree, state licensure, and experience supervising assistants.

Qualifications

• Graduated from an accredited Physical Therapy program and licensed in the state.
• Experience supervising Physical Therapy Assistants.
• Solid documentation and charting skills (evaluations, progress notes, discharge docs).
• Experience in long-term care or geriatric settings preferred.
• Punctual, professional, and able to collaborate with multidisciplinary team.
• HIPAA compliance and patient confidentiality basics.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment programs with goals aligned to patient capacity and tolerance.
• Set realistic, achievable goals for patients and document status.
• Educate patients and families on physical therapy methods and expectations.
• Collaborate with all disciplines to provide patient-centered care.
• Perform duties as assigned by Director of Rehabilitation and adhere to confidentiality guidelines.
