Job summary

The Gallatin Center for Rehabilitation and Healing is seeking a Physical Therapist (PT) for a PRN weekend position. The PT will evaluate referred patients, develop targeted treatment plans, and deliver direct patient care within a geriatric long-term care setting.

Responsibilities include documenting patient status, updating progress notes, coordinating with the care team, and educating patients and families on therapy goals.

Qualifications

• Graduate from an accredited Physical Therapy program.
• Must hold a valid PT license and state registration.
• Oversee and supervise Physical Therapy Assistants.
• Experience in long-term care/geriatric care preferred.
• Strong documentation skills.
• Completes evaluations, progress notes, recertification and discharge docs.
• Collaborates with all disciplines for patient-centered care.
• Demonstrates exceptional patient service and clinical excellence.
• Punctual, adheres to scheduled hours, maintains professionalism.
• Performs duties assigned by Director of Rehabilitation.
• Complies with confidentiality, HIPAA and Residents' Rights.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment programs with goals per patient capacity.
• Document patient condition and educate patients and families on therapy methods.
