Job summary

The Gallatin Center for Rehabilitation and Healing is seeking a Physical Therapist (PT) to support weekend staffing in our facility. The PT will assess patients, develop and implement treatment plans, and document progress under physician direction.

Requirements include graduation from an accredited PT program, active licensure, and experience supervising PT assistants. Strong documentation and patient-centered care are essential.

Qualifications

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of practice.
• Overseeing and supervising Physical Therapy Assistants.
• Experience with long-term care and/or geriatric care preferred.
• Well versed with documentation writing.
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide patient-centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence.
• Punctuality, professionalism and adherence to scheduled hours.
• Perform all other duties assigned by Director of Rehabilitation.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment programs with goals suited to capacity and tolerance under physician direction.
• Set realistic goals, document patient condition and educate patients and families on physical therapy methods.
