Job summary

The Gallatin Center for Rehabilitation and Healing is seeking a Physical Therapist (PT) to join our facility on a PRN basis, with emphasis on weekend staffing. The PT will assess patients, develop and manage treatment plans, and educate families as part of comprehensive geriatric care.

Responsibilities include supervising PT Assistants, completing evaluations and progress notes, and ensuring confidentiality and compliance with HIPPA and Residents Rights. Competitive benefits offered.

Qualifications

• Graduated from an accredited Physical Therapy college or university.
• Valid professional license and registration for state of practice.
• Overseeing and supervising Physical Therapy Assistants.
• Experience with long-term care and/or geriatric care preferred.
• Strong documentation writing skills.
• Evaluations, weekly progress notes, recertification and discharge documentation.
• Collaborates with all disciplines to provide best patient-centered treatment and care.
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer a treatment program with goals according to patient capacity and tolerance.
• Document and record patient condition and educate patients and families in PT methods.
