Job summary

The Gallatin Center for Rehabilitation and Healing is seeking a Physical Therapist (PT) for PRN weekend staffing at our facility in Hendersonville, TN. Primary need is weekend coverage, with consideration for additional availability.

The PT will assess referred patients, develop and supervise treatment plans, educate patients and families, and document progress while collaborating with the care team. Licensure in Tennessee and facility experience are preferred.

Qualifications

• Graduated from an accredited Physical Therapy program.
• Valid PT license/registration in the state.
• Experience supervising Physical Therapy Assistants.
• Experience in long-term care/geriatric settings preferred.
• Proficient in documentation writing and patient progress notes.
• Ability to perform evaluations, discharge planning, and recertifications.
• Collaborates with other disciplines to provide patient-centered care.
• Maintains professionalism, punctuality, and confidentiality (HIPAA).

Responsibilities

• Assess referred patients and provide direct PT care.
• Develop and administer treatment programs per physician direction.
• Set realistic goals and educate patients and families.
• Document condition and progress; maintain patient records.
• Supervise Physical Therapy Assistants.
• Collaborate with other disciplines to deliver care.
• Adhere to confidentiality and Residents' Rights.
