Job summary

Gallatin Center for Rehabilitation and Healing is seeking a Physical Therapist (PT) for weekend staffing on a PRN basis at our facility in Tennessee. The PT will assess patients, design treatment plans, supervise assistants, and coordinate with the care team to deliver patient-centered rehabilitation.

Strong documentation, geriatrics experience, and Tennessee licensure are required. The role emphasizes quality patient care, compliance, and professional growth within our facility.

Qualifications

• Must have graduated from an accredited Physical Therapy college or university.
• Must hold a valid Physical Therapist license and Tennessee registration.
• Experience supervising Physical Therapy Assistants and collaborating with care teams.

Responsibilities

• Assess referred patients and determine rehabilitation needs.
• Develop and administer individualized treatment plans with goals.
• Document patient progress, evaluations, and discharge planning.
• Educate patients and families on therapy methods and expectations.
• Supervise Physical Therapy Assistants and coordinate with the team.
• Ensure confidentiality and HIPAA compliance in all records.
• Maintain punctual attendance and professional conduct as required by Rehabilitation Director.
