The Gallatin Center for Rehabilitation and Healing is seeking a Physical Therapist (PT) for PRN weekend staffing. The role focuses on direct patient care, assessments, and goal-driven rehabilitation for residents in long-term care settings.

The PT will develop and administer treatment plans, document progress, educate patients and families, and supervise Physical Therapy Assistants while collaborating with the rehabilitation team. Equal opportunity employer.

Qualifikationen

• Graduated from an accredited Physical Therapy program.
• Current state license/registration to practice.
• Experience supervising Physical Therapy Assistants.
• Experience in long-term/geriatric care preferred.
• Strong documentation and record-keeping.
• Independent evaluator with discharge planning.
• Collaborates with other disciplines to deliver patient-centered care.
• Demonstrates professionalism and punctuality.
• Maintains patient confidentiality and HIPAA compliance.

Aufgaben

• Assess referred patients needing rehabilitation.
• Provide direct patient care and set treatment goals.
• Develop and administer individualized treatment programs.
• Document patients' condition and progress.
• Educate patients and families.
• Oversee PT Assistants and supervise staff.
• Ensure confidentiality and compliance.
• Perform other duties as assigned by Director of Rehabilitation.
