Job summary

The Gallatin Center for Rehabilitation and Healing is seeking a Physical Therapist for PRN weekend staffing. The role involves assessing referred patients, establishing and administering treatment plans, and setting realistic goals with physician direction to support resident rehabilitation and independence.

The candidate must graduate from an accredited PT program, hold a valid license, and supervise Physical Therapy Assistants.

Qualifications

• Graduate from an accredited Physical Therapy college or university.
• Valid state license and registration.
• Oversee and supervise Physical Therapy Assistants.
• Experience with long-term care and/or geriatric care preferred.
• Strong documentation writing skills.
• Experience with evaluations, weekly progress notes, recertification and discharge documentation.
• Collaborates with all disciplines to provide patient-centered care.
• Exceptional patient service and clinical excellence.
• Punctual, professional, and reliable in scheduled hours.
• Duties assigned by Director of Rehabilitation.
• Confidentiality and HIPAA compliance.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment programs with goals aligned to patient capacity and physician direction.
• Document and record patient status and educate patients/families in PT methods.
• Supervise Physical Therapy Assistants and collaborate with other disciplines.
