Job summary

The Gallatin Center for Rehabilitation and Healing is seeking a Physical Therapist (PT) to join our facility on a PRN basis, with primary weekend staffing needs; other availability will be considered to support residents.

You will evaluate and treat referred patients, develop and document care plans, supervise PTAs, collaborate with the care team, and educate patients and families. Licensed PT in good standing required; experience in long-term care preferred.

Qualifications

• Must be licensed as a Physical Therapist in the state of practice.
• Graduated from an accredited Physical Therapy program.
• Experience supervising Physical Therapy Assistants.
• Experience in long-term care/geriatric settings preferred.
• Strong documentation and record-keeping abilities.
• Collaborates with other disciplines to provide patient-centered care.

Responsibilities

• Assess referred patients and provide direct PT care to evaluate condition and function.
• Develop and administer treatment programs with goals based on patient capacity.
• Document patient progress and discharge information accurately.
• Educate patients and families on therapy techniques and home programs.
• Supervise and instruct Physical Therapy Assistants.
• Collaborate with the care team to deliver comprehensive rehabilitation.
