Job summary

The WellFund, LLC is seeking a Patient Eligibility Coordinator to support patients at partner hospital facilities on weekends. You will guide patients through Medicaid, Medicare, and ACA Marketplace options, and collaborate with hospital teams to ensure enrollment is timely and accurate.

Qualifications

• High school diploma or equivalent is required.
• Strong desire to help underserved populations and patients with social determinants of health.
• Excellent communication and interpersonal skills.
• Ability to work independently and collaboratively in a fast-paced hospital environment.

Responsibilities

• Assist patients in understanding and applying for appropriate healthcare coverage programs.
• Guide and support patients through enrollment for Medicaid, Medicare, and ACA Marketplace plans.
• Educate patients on options and eligibility based on program requirements.
• Collaborate with hospital Patient Access and Case Management teams to ensure continuity of care.
• Stay informed of updates in healthcare programs and policies.
• Maintain accurate documentation and follow all compliance protocols.
