Job summary

The WellFund, LLC seeks a Patient Eligibility Coordinator to work with hospital partners on weekends. The role focuses on guiding patients through coverage options and helping them access care with empathy.

You will collaborate with Patient Access and Case Management, educate patients on Medicaid, Medicare, and ACA Marketplace plans, and maintain accurate documentation while staying current on program changes.

Qualifications

• High school diploma or equivalent required; healthcare related coursework preferred.
• Strong desire to help underserved populations and patients facing social determinants of health.
• Excellent communication and interpersonal skills.
• Ability to work independently and collaboratively in a fast-paced hospital environment.
• Willingness to learn and adapt to evolving healthcare programs and policies.

Responsibilities

• Assist patients in understanding and applying for appropriate healthcare coverage programs.
• Guide enrollment for Medicaid, Medicare, and ACA Marketplace plans.
• Educate patients on options and eligibility based on program requirements.
• Collaborate closely with hospital Patient Access and Case Management teams to ensure continuity of care.
• Stay informed of updates and changes in healthcare programs and policies.
• Maintain accurate documentation and follow all compliance protocols.
