Job summary

Reserve Care, formerly Hospice of the Western Reserve, is seeking a Weekend Registered Nurse (RN) to provide home-based hospice nursing services in Willoughby Hills, OH. Schedule: Saturdays and Sundays, 8:00 am–8:30 pm. Active Ohio RN licensure and compassionate patient-focused care are required.

You will assess needs, manage symptoms, educate patients and families, and collaborate with physicians to support end-of-life care with dignity and respect.

Qualifications

Licensed RN in Ohio state.
Experience with pain and symptom management preferred.
Ability to work weekends and travel to patient homes as needed.
Strong communication and documentation skills.

Responsibilities

Assess physical, psychosocial, and spiritual needs of patients and families.
Provide intermittent skilled nursing services under physician direction.
Monitor pain and symptoms and educate families to manage them.
Liaise with physicians and coordinate care across teams.
Provide emotional support and compassionate care at end of life.
Document promptly and accurately; maintain confidentiality.
