Qualifications

• Must have a criminal background check.
• One or more years of experience preferred in community/home health agency or hospital.

Responsibilities

• Perform Hospice visits as needed during weekend hours.
• Evaluate the effectiveness of nursing service to the patient and family.
• Prepare and present patient's record to the Clinical Record Review Committee.
