Job summary

The Care Team Hospice is seeking a Weekend On-Call Registered Nurse to provide skilled nursing support, symptom management, and crisis intervention for hospice patients in homes and facilities. This role blends independence, field work, and collaboration with an interdisciplinary team, with dedicated weekend coverage and after-hours responsiveness.

You will perform on-call visits, manage symptoms, educate families, and document care activities in the EMR, while upholding regulatory requirements

Qualifications

• Active RN license in the state of practice.
• Hospice, home health, oncology, palliative care, or critical care experience preferred.
• Strong clinical assessment and communication skills.

Responsibilities

• Respond to after-hours and weekend patient, family, and caregiver needs with timely nursing assessments and interventions.
• Perform on-call visits to assess changes in patient condition and adjust care plans.
• Coordinate care with physicians, caregivers, pharmacies, and the interdisciplinary team to ensure continuity of care.
