The Care Team Hospice in Illinois is seeking an On-Call Registered Nurse to support patients in homes and facilities, with a focus on symptom management and crisis intervention. This role blends independence with collaboration within an interdisciplinary team.

Primarily remote when not responding to patient needs, the position offers flexible scheduling including weekend or weekday on-call shifts, and opportunities for professional growth and advancement within a supportive environment.

Qualifications

• Active RN license in state of practice required.
• Minimum of 1 year nursing experience; hospice/home health/oncology/palliative care/critical care preferred.
• Strong clinical assessment and symptom management skills.
• Ability to work independently and make sound clinical decisions in the field.
• Experience with EMR; HCHB preferred.

Responsibilities

• Respond to after-hours and weekend patient, family, and caregiver needs with timely nursing assessments and interventions.
• Perform on-call visits to assess changes in patient condition, manage symptoms, and update care plans.
• Coordinate care with physicians, caregivers, pharmacies, and the interdisciplinary team to ensure continuity of care.
• Educate patients, families, and caregivers about medications, symptom management, and end-of-life care.
• Facilitate admissions, deaths, and urgent clinical situations per hospice policies.
• Document patient assessments and care coordination accurately in the EMR.
• Deliver compassionate, patient-centered care supporting physical, emotional, and psychosocial needs.
