Job summary

Luminos Hospice is seeking a Weekend On-Call Hospice RN to provide compassionate care to patients in their homes on weekends. You will coordinate with the interdisciplinary team and families to ensure continuity of care and adherence to care plans.

Qualified candidates hold an active RN license, CPR certification, and have two years of clinical nursing experience with one year in hospice preferred. Valid driver's license and reliable transportation required for travel to patient residences.

Qualifications

• Must hold a current RN license in the state.
• Two (2) years of nursing experience in a clinical care setting and one (1) year full-time hospice preferred.
• Knowledge of state and federal regulations; ability to work independently and make sound decisions.

Responsibilities

• Administer nursing care for patients in their place of residence.
• Coordinate care with the Interdisciplinary Group/Team, patients, families, and referring agency.
• Participate in coordination of care and ensure quality patient outcomes.
• Communicate clearly with families and team members to support care plans.
