Job summary

Luminos Hospice is seeking a skilled On-Call RN to join our weekend team, providing nursing care for patients in their homes and coordinating with the Interdisciplinary Group/Team, patients, families, and referring agencies.

This role requires a valid State RN license, current CPR certification, and at least two years of clinical nursing experience with hospice preferred. Friday 5pm–Monday 8am, with comprehensive benefits and generous PTO.

Qualifications

• Current State RN license.
• Current driver’s license.
• Current CPR Certification.
• Two years' nursing experience in a clinical setting and one year full-time hospice preferred.

Responsibilities

• Provide nursing care for patients in their home and coordinate care with the Interdisciplinary Group/Team, patients, families, and referring agency.
• Administer nursing care during weekends and ensure smooth handoffs and documentation.
• Maintain high standards of clinical practice and patient safety according to state and federal regulations.
