Job summary

Trinity Health Group LLC is seeking an On-Call Registered Nurse to support our hospice team with after-hours care and weekend coverage. The role involves responding to calls, triaging needs, and performing home visits to ensure quality, compassionate patient care.

The RN will collaborate with the interdisciplinary team, document visits accurately, and provide emotional support to patients and families during crises.

Qualifications

• Current, active RN license in Texas.
• Minimum of one year nursing experience (hospice or palliative care preferred).
• Strong clinical communication and advocacy skills.

Responsibilities

• Provide on-call hospice nursing support during night hours.
• Conduct home visits for urgent patient needs, symptom management, or pronouncement of death.
• Collaborate with the interdisciplinary hospice team to ensure continuity of care.
• Document all patient interactions accurately and in a timely manner.
• Offer emotional and clinical support to patients and families in crisis situations.
