Trinity Health Group LLC is seeking an On-Call Registered Nurse (RN) for weekend night shifts to support hospice patients and families after hours.

The role includes responding to calls, triaging needs, and performing home visits to ensure high-quality, compassionate care. Candidates should have Texas RN licensure, hospice experience, and strong communication and analytical skills.

Qualifications

• Current, active RN license in Texas.
• Minimum of one year nursing experience (hospice or palliative care preferred).
• Strong clinical assessment and critical thinking skills.
• Compassionate communication and patient advocacy skills.
• Reliable transportation, auto insurance and valid driver’s license.

Responsibilities

• Provide on-call hospice nursing support during night hours.
• Conduct home visits for urgent patient needs, symptom management, or pronouncement of death.
• Collaborate with the interdisciplinary hospice team to ensure continuity of care.
• Document all patient interactions accurately and in a timely manner.
• Offer emotional and clinical support to patients and families in crisis situations.
