Job summary

Trinity Health Group LLC is seeking an On-Call Registered Nurse (RN) to join our hospice team in Texas. This weekend night position supports patients and families after hours, responding to calls, triaging needs, and visiting homes as needed to ensure high-quality, compassionate care.

The role requires an active Texas RN license, at least one year of nursing experience (hospice preferred), strong assessment skills, and the ability to provide comforting, patient-centered support in crisis

Qualifications

• Current, active RN license in Texas.
• Minimum of one year nursing experience (hospice or palliative care preferred).
• Strong clinical assessment and critical thinking skills.
• Compassionate communication and patient advocacy skills.
• Reliable transportation, auto insurance and valid driver’s license.

Responsibilities

• Provide on-call hospice nursing support during night hours.
• Conduct home visits for urgent patient needs, symptom management, or pronouncement of death.
• Collaborate with the interdisciplinary hospice team to ensure continuity of care.
• Document all patient interactions accurately and in a timely manner.
• Offer emotional and clinical support to patients and families in crisis situations.
