Job summary

Qualifications

• Graduate of an accredited school of nursing with current RN licensure in Ohio.
• Demonstrates time management, multitasking, and independent working capabilities.
• CPR certification is required.

Responsibilities

• Coordinate and provide direct care/support for hospice patients in home and facility settings.
• Respond to on-call needs during nights and weekends with appropriate documentation and care coordination.
• Collaborate with the hospice interdisciplinary team to manage admissions, discharges and care transitions.
