The Pennant Group is seeking a Weekend On-Call Registered Nurse to provide home health and hospice services on Saturdays and Sundays from 8:00 AM to 5:00 PM. The role emphasizes community health education and patient-centered care in home settings.

Qualified RNs will hold current Utah licensure, CPR certification, and prefer 2+ years in home health or hospice. This on-call position requires strong assessment, care planning, and coordination with the clinical team.

Qualifikationen

• Active RN license in Utah and current CPR certification.
• Graduate of an accredited nursing program with licensure to practice in-state.
• Minimum two years' experience in home health, hospice, or public health preferred.

• Conduct initial and ongoing patient and family assessments to determine hospice needs.
• Provide professional nursing care using the nursing process and evidence-based practices.
• Contribute to the plan of care and revise as necessary in collaboration with the care team.
• Administer medications and treatments as prescribed and monitor patient responses.
• Educate patients and families on self-care, medications, diets, and disease management.
• Maintain accurate documentation of notes, changes in condition, and care provided.
• Fulfill on-call duties, coordinate equipment and services, and supervise home health aides when needed.
• Coordinate with interdisciplinary team to ensure continuity of care and support for patient independence.
