Job summary

Canyon Home Care & Hospice is seeking a Full-time on-call RN to join our caring team. The role includes evenings and nights, weekends, and some holidays with travel within Salt Lake County. We offer competitive pay, generous PTO, and comprehensive health benefits.

Responsibilities include triaging calls, providing skilled nursing care, and coordinating in-home hospice services. CPR is required, along with an active Utah RN license and reliable transportation.

Qualifications

Experience in home health or hospice settings is required.
Strong triage and critical thinking skills are essential.
Proficient in navigating electronic medical records (EMR).
Cheerful, customer-service oriented attitude is expected.
Excellent availability to respond to calls during on-call shifts.
Reliable transportation and auto insurance are required.

Responsibilities

Provide on-call nursing coverage during evenings, nights, weekends and some holidays.
Triage patient needs over the phone and coordinate care as needed.
Travel within Salt Lake County for patient visits and in-home care as assigned.
