Job summary

Ascend Health is seeking a compassionate Registered Nurse (RN) for a part-time on-call role in Northern Utah. You will provide after-hours triage, nursing visits, and family support in patients' homes, coordinating with the interdisciplinary team to ensure timely responses and continuity of care.

Responsibilities include skilled nursing visits after hours, prompt triage, and accurate documentation in the EMR.

Qualifications

• Current Registered Nurse (RN) license in Utah.
• Minimum of 1 year nursing experience (Home Health or Hospice preferred).
• Strong clinical assessment and communication skills.

Responsibilities

• Provide skilled nursing visits for urgent needs, admissions, or symptom management after hours.
• Respond promptly to patient and family calls to provide triage and support.
• Communicate effectively with physicians, team members, and oncoming staff.
• Maintain accurate and timely documentation in the EMR.
• Deliver compassionate care aligned with Ascend Health's mission.
