Agape Hospice Care seeks a passionate PRN On-Call Registered Nurse (RN) to join our hospice care team. Primary duties include delivering skilled nursing care in patients’ homes, coordinating with the interdisciplinary team, families, and referring agencies to ensure seamless care delivery.

The position emphasizes weekend on-call responsibilities with weekend shifts, excellent patient advocacy, and strong communication while supporting patients and families through hospice services.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Two years’ RN experience in a clinical setting; hospice or home health preferred.
• Current state RN license and driver’s license.
• Current CPR certification.
• Proficient computer skills; Microsoft Office experience preferred.

Responsibilities

• Administer skilled nursing care for clients in their place of residence.
• Coordinate care with the inter-disciplinary team, patient/family and referring agency.
