Angels Care Home Health in Omaha, NE is seeking a Weekend On-Call Licensed Practical Nurse (LPN). You will work under the direction of an RN to assess patient needs, participate in planning and delivering home health care, and communicate with patients and care teams.

The ideal candidate is a graduate of an accredited practical nursing program, holds a current LPN license in Nebraska, and maintains CPR and Hepatitis profile. A valid driver's license is required to support community visits.

Formación

• Must be a graduate of an accredited LPN program.
• Current LPN/LVN license in the state.
• Current CPR certification and Hepatitis profile.
• Driver's license required.

Responsabilidades

• Under RN direction, assess patient needs as evidenced by documentation and evaluations.
• Plan and implement care with RN based on the POC.
• Provide effective communication to patients, staff and referrals.
• Monitor cases for third-party payer requirements.
• Ensure continuity of care through delivery of quality patient care.

Functions:

• Under the direction of a RN, assist in identifying the patient’s physical, psycho social, and environmental needs as evidenced by documentation, clinical record, case conference, team report, and evaluations.
• Participate in planning and implementing care in conjunction with the RN, in accordance with the POC.
• Provide effective communication to patients, staff members, other health care professionals, and referral sources.
• Monitor assigned cases to ensure compliance with requirements of third party payer.
• Assure continuity of care through delivery of quality patient care.

Qualifications:

• Graduate of an accredited school of practical/vocational nursing.
• Current state license as a License Practical/Vocational Nurse.
• Current State’s Driver’s License.
• Two years’ experience as an LPN/LVN, home health preferred.
• Proof of current CPR and Hepatitis profile.
