Job summary

Angels Care Home Health is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care in clients' homes under RN supervision. The role focuses on coordinating with the interdisciplinary team, patient, family, and referral sources to ensure quality care.

Applicants must have an approved LPN program, active state license, and a valid driver’s license; CPR and Hepatitis profile current. Two years of LPN experience, preferably in home health, is desired.

Qualifications

• Graduate of an accredited school of practical/vocational nursing.
• Current state license as an LPN.
• Current State’s Driver’s License.
• Two years’ experience as an LPN, home health preferred.
• Proof of current CPR and Hepatitis profile.

• The primary function of the LPN is to administer skilled nursing care, under the supervision of a registered nurse, for clients of all ages in their place of residence. Coordinates care with the interdisciplinary team, patient/family, and referring agency.
