Job summary

Angels Care Home Health is seeking a Licensed Practical Nurse to provide skilled nursing care in patients' homes under the supervision of a registered nurse. The role includes coordinating care with the interdisciplinary team, patients and families, and referring agencies to ensure optimal outcomes.

Qualified candidates will hold an LPN license, a valid driver's license, CPR and Hepatitis profile, and typically have at least two years of LPN experience with home health.

Qualifications

• Graduate of an accredited school of practical/vocational nursing.
• Current state LPN license.
• Valid driver’s license.
• Two years’ experience as an LPN; home health preferred.
• CPR and Hepatitis profile current.

Responsibilities

• The primary function is to administer skilled nursing care under RN supervision in clients' homes.
• Coordinate care with the interdisciplinary team, patient/family, and referring agency.

About the position:
