Angels Care Home Health is seeking an Licensed Practical Nurse (LPN) to provide skilled nursing care in clients' homes under RN supervision. You will coordinate with the care team, patients, and families to deliver high-quality home health services.

Qualifications

• Graduate of an accredited practical/vocational nursing program.
• Current state LPN license.
• Current state's driver's license.
• Two years of LPN experience; home health preferred.

About the position:

The primary function of the LPN is to administer skilled nursing care, under the supervision of a registered nurse, for clients of all ages in their place of residence. Coordinates care with the interdisciplinary team, patient/family, and referring agency.

Qualifications:

• Graduate of an accredited school of practical/vocational nursing.
• Current state license as an LPN.
• Current State’s Driver’s License.
• Two years’ experience as an LPN, home health preferred.
• Proof of current CPR and Hepatitis profile.
