Job summary

Angels Care Home Health in Arizona is seeking a licensed practical nurse (LPN) to provide skilled nursing care in clients’ homes under RN supervision. You will coordinate with a multidisciplinary team, patients and families to ensure high-quality care and continuity of services.

The role requires an active LPN license in the state, a valid driver’s license, and current CPR and Hepatitis profile. Angels Care offers extensive training, comprehensive benefits, PTO, and opportunities for advancement.

Qualifications

• Graduate of an accredited school of practical/vocational nursing.
• Current state license as an LPN.
• Current State’s Driver’s License.
• Two years’ experience as an LPN, home health preferred.

About the position:

The primary function of the LPN is to administer skilled nursing care, under the supervision of a registered nurse, for clients of all ages in their place of residence. Coordinates care with the interdisciplinary team, patient/family, and referring agency.
