This full-time role supports the home health team and coordinates with hospitals, rehab facilities, and the scheduling department to ensure timely patient onboarding.

Ideal candidates will have experience in intake or care coordination, be comfortable with EMR systems, and demonstrate strong communication and organizational skills.

Qualifications

• Experience in intake, home health, medical office, scheduling, or care coordination.
• Comfortable working with electronic medical record systems.
• Compassionate, patient-centered approach.
• Able to work independently and manage multiple priorities.

Responsibilities

• Process and finalize new patient referrals in Homecare Homebase.
• Complete patient welcome calls and document coordination notes.
• Follow up on pending and hold patients; contact hospitals/rehab facilities for updates.
• Complete pending and hold reports for assigned districts.
• Assist scheduling team for smooth transition into home health care.
• Provide clerical support to the Nursing Supervisor.
• Answer incoming calls and connect callers to the right resource.
