Job summary

AdventHealth in Leesburg, FL is seeking an Intake Coordinator for the Home Health Office. You will coordinate the intake of referrals, verify physician licensure for Medicare participation, and act as the liaison between hospitals, offices and clinical teams to schedule patient care.

The role requires attention to detail, strong customer service and computer skills, with part-time hours including weekend days at the Leesburg location.

Qualifications

• Knowledge of Medicare billing processes, privacy regulations and Medicare home health regulations [Preferred]
• Proficient in use of personal computer, copier, phone, facsimile machines [Required]
• High level of multitasking, organization and proactive communication skills [Required]
• Moderate – advanced computer skills [Required]
• Strong customer service skills [Required]

Responsibilities

• Coordinates the intake of all referrals into the agency.
• Serves as the central coordination point for all referrals coming from hospitals, physician offices and any other referral source.
• Verifies that physicians ordering homecare are licensed and able to participate in the Medicare program.
• Performs care coordination activities upon receipt of patient referral; assesses if homecare services are appropriate, verify/obtain physician orders, ensure payer coverage is verified, coordinates arrangements for HME, infusion and other needs.
• Assists with weekly coverage rotation to ensure scheduling function is completed appropriately when staff off.
