Job summary

VNA of Florida is seeking a Weekend Intake Coordinator in Stuart, FL. This in-office role runs Saturdays and Sundays from 8:00 AM to 5:00 PM with a 1-hour lunch, supporting patient referrals, welcome calls, and care coordination in home health.

You will work closely with the scheduling team and nursing supervisor to ensure smooth transitions for patients. The ideal candidate has strong communication and organizational skills, multitasking ability, and experience with EMR systems.

Qualifications

• Clear communication with patients, families, and partners.
• Excellent organization and attention to detail.
• Ability to juggle multiple tasks in a fast-paced intake environment.
• Experience with intake, home health, scheduling, or care coordination helps.
• Familiarity with EMR platforms and documentation processes.

Responsibilities

• Process and finalize new patient referrals in the EMR system.
• Complete patient welcome calls and document coordination notes.
• Follow up on pending patients and coordinate with hospitals and facilities.
• Prepare pending and hold reports for assigned districts.
• Collaborate with scheduling to ensure smooth transitions to home health care.
• Provide clerical support to the Nursing Supervisor.
• Answer phone calls and route inquiries to the right resource.
