Vivie is seeking a Clinical Intake Coordinator to manage referrals end-to-end, verify patient data, and support timely care initiation. The role emphasizes HIPAA-compliant data handling, cross-functional collaboration, and serving as a compassionate front-line contact for clients and professionals.

Qualifications

• LPN license in Minnesota and North Dakota is required.
• Excellent organizational and data-entry skills are essential.
• Strong communication and customer-service abilities are required.
• Ability to pass state background checks and maintain HIPAA compliance.

Responsibilities

• Manage end-to-end referral process and assess service eligibility.
• Nurture referrals and follow up with patients and sources.
• Ensure accurate documentation and EMR data entry.
• Collaborate with Sales, Outreach, and operations teams.
• Serve as a first point of contact with empathy and guidance.
• Perform other duties as assigned.
