Job Summary

The Intake Coordinator manages all aspects of patient intake, including communicating with patients and families, data entry, maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, facilitating daily intake stand‑up meetings, and overseeing insurance verification and authorization processes.

Responsibilities

• Direct all daily patient referral and intake operations, providing oversight of intake policy development and implementation.
• Ensure compliance with state, federal, and Joint Commission referral/intake regulatory requirements.
• Implement improved work methods and procedures to guarantee patient admission in accordance with policy.
• Establish and maintain positive working relationships with current and potential referral sources.
• Ensure seamless transition of patients to home care by overseeing patient education, preparation for home care, plan of care initiation, and coordination of care with multiple service providers.
• Maximize third‑party reimbursement through direct oversight of insurance verification and authorization processes.
• Build and monitor community and customer perceptions of the agency as a high‑quality provider of services.
• Gather, collate, and report referral statistics, including key customer referral trends.
• Maintain comprehensive knowledge of agency contractual relationships and ensure patient admission according to contract provisions.
• Maintain comprehensive knowledge of community resources and assist referral sources in accessing those resources when services are not provided by the agency.

Qualifications

Compensation

Pay Range: $23.00 – $28.00 per hour
