Job summary

21st Century Home Health Services is seeking a licensed CA RN to provide home health care across multiple counties in California. You will conduct SOC admissions, follow-up visits, and in-home assessments, delivering high-quality patient care in collaboration with a clinical team.

Responsibilities include wound care, IV infusions per protocol, medication reconciliation, and accurate documentation. A valid CA driver's license and BLS/CPR are required, with opportunities for professional growth

Qualifications

• Current and unrestricted CA RN license.
• BLS/CPR Certification.
• Valid CA Driver's License.
• Experience with SOC/OASIS is highly desired.
• Experience with IV infusions is preferred.

Responsibilities

• Conduct Start of Care (SOC) Admissions and OASIS assessments.
• Perform follow-up visits and evaluations in designated territory.
• Make home visits to patients in designated territory.
• Reconcile medications, wound care, treatments, and procedures.
• Monitor patient conditions and report changes to the clinical team.
• Initiate and administer IV medication infusions per protocol.
• Document observations and care interventions accurately.
