Job summary

21st Century Home Health Services (21HHS) is seeking a Part-time Registered Nurse Case Manager for Lincoln/Northern Sacramento area to perform SOC admissions, follow-ups, and in-home visits. You will reconcile meds, manage IV infusions, and collaborate with physicians to deliver high-quality home health care.

The role requires CA RN license, BLS/CPR, a valid CA driver’s license, and strong assessment skills within a supportive, growth-oriented team.

Qualifications

• Current and unrestricted CA RN license.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Experience conducting assessments and Start of Care (SOC) - OASIS is highly desired.
• High level of clinical knowledge in home health care.

Responsibilities

• Perform Start of Care and SOC admissions with OASIS as applicable.
• Conduct follow-up and evaluation visits in designated territory.
• Make home visits to patients in Lincoln/Northern Sacramento area.
• Reconcile medications, wound care, treatments and procedures.
• Monitor patient conditions and report changes to clinical team.
• Collaborate with physicians and staff to deliver quality care.
• Initiate IV medication infusions per protocol and document care.
