Job summary

Reserve Care in Cleveland, OH is seeking a Care Navigation Social Worker to support referral intake and psychosocial assessment in end-of-life care. The role focuses on access facilitation, one-call resolution, and coordinating with an interdisciplinary team.

Shifts are Saturdays and Sundays, 8:00am–8:30pm, totaling 24 hours weekly (paid for 28). LSW in Ohio required; hospice experience preferred; strong communication and multitasking skills are essential.

Qualifications

• Licensed Social Worker (LSW) in Ohio required.
• Hospice experience preferred.
• Strong multitasking abilities and communication skills.
• Ability to work effectively within an interdisciplinary team.

Responsibilities

• Assist with referral intake and processing through dispatch using multiple technology platforms.
• Support psychosocial assessment of complex and challenging referrals.
• Follow up on referrals to ensure access, quality, and timeliness.
• Participate in quality improvement and process improvement initiatives.
• Train peers on referral/intake/dispatch and end-of-life topics.
• Perform all other duties as assigned.
