Job summary

Reserve Care is seeking an experienced Care Navigation Nurse (LPN) in Ohio to support referral intake, coordination, and initial triage for hospice services. The role emphasizes timely access and one-call resolution wherever possible.

The ideal candidate will have an active Ohio LPN license, hospice experience, and excellent communication and teamwork skills to support patients, families, and care teams in challenging situations.

Qualifications

• Excellent verbal and written communication skills.
• Ability to communicate calmly and compassionately with patients and families in distress.
• Hospice experience strongly preferred.

Responsibilities

• Support referral intake, care coordination, and initial clinical triage for hospice services.
• Schedule appointments and manage the dispatch process for prospective patients.
• Take clinical calls from patients and caregivers and escalate as needed.
• Notify primary care teams of relevant clinical issues.
• Participate in quality and process improvement initiatives.
