Job summary

Reserve Care (formerly Hospice of the Western Reserve) is seeking a Navigator Social Worker to provide psychosocial support to patients and families in a comprehensive care program. You will collaborate with a transdisciplinary team to assess needs, coordinate resources, and assist with financial and placement arrangements to enhance quality of life during advanced illness.

The role requires Ohio social work licensure, at least one year of relevant experience, and strong communication skills,

Qualifications

• Must hold a social work license for the State of Ohio.
• Minimum of one year of social work experience in a recognized health care organization.
• Knowledge of hospice and palliative care standards and procedures.
• Ability to work within NASW ethics and professional guidelines.
• Strong written and verbal communication skills.

Responsibilities

• Provide psychosocial support to Navigator patients and families.
• Coordinate with the transdisciplinary team to plan care and arrange services.
• Assist with placement and coordination of financial resources for patients.
• Document care activities following organization standards and regulatory requirements.
• Support patient–family decision making and goal setting through counseling.
