Job summary

Qualifications

• Master's Degree in Social Work from an CSWE-accredited program.
• Current Ohio LSW licensure.
• Two years of varied work experience as an MSW.
• BLS certification through AHA or Red Cross.
• Knowledge of JCAHO and Medicare standards and regulations.
• Ability to build relationships with patients, physicians, and community agencies.

Responsibilities

• Process inpatient referrals and coordinate discharge planning across facilities.
• Assess, plan, and provide patient care per the plan of care.
• Coordinate care while visiting hospice patients and families.
• Document in the electronic medical record (EMR) and ensure accurate records.
• Provide symptom management and support to maintain patient comfort.
• Collaborate with the interdisciplinary team to adjust plans as patient status changes.
• Coordinate care for a caseload across inpatient and community settings.
