Hospice of San Joaquin is seeking a Weekend, Part-Time RN to provide skilled nursing assessments and care coordination for patients and families. The role supports on-call and off-hour coverage, delivering pain and symptom management, crisis intervention, and education in alignment with the IDT plan of care.

The ideal candidate will have a California RN license, hospice or home health experience, strong clinical judgment, and excellent communication skills.

Qualifications

• Must hold active California RN license.
• One year of recent clinical experience in acute care preferred; hospice/home health preferred.
• Ability to set priorities, organize, and work independently.
• Ability to make independent clinical decisions.
• Knowledge of medications including indications, dosing, side effects and toxicity.
• Solid hospice nursing knowledge and philosophy alignment.
• Strong verbal and written communication skills.
• CPR certification required.
• Pre-employment drug screening required.

Responsabilités

• Provide skilled nursing assessment and care delivery during weekend hours.
• Support pain and symptom management, crisis intervention, and family education by phone or home visits.
• Document and report to regular staff with clear problem-action-goal statements.
• Coordinate with IDT members and other providers to ensure appropriate level of care.
• Assist with end-of-life support for patients and caregivers.
• Drive care coordination with facilities as needed and supervise staff during off-hours.
