Job summary

AccentCare in San Diego County is seeking an RN for weekend on-call hospice nursing. This full-time role covers Friday 5pm–8am, Saturday 8pm–8am, Sunday 8pm–8am, serving as the on-call admission RN.

You will perform comprehensive assessments, coordinate with IDG, educate patients and families, and ensure timely documentation. Lifting patients and equipment is required; proficiency with EMR and MS Office is expected.

Qualifications

• Current RN licensure in the applicable state is required; associate degree preferred.
• CPR certification is recommended; required for direct patient care in Georgia.
• Experience in hospice, home health, medical-surgical, geriatrics or pediatrics.
• Willingness to participate in on-call rotation; reliable transportation and valid driver's license.
• Strong communication and organizational skills; proficient in MS Office and EMR; teamwork within IDG.
• Ability to lift patients and move equipment; frequent standing/walking.

Responsibilities

• Conducts comprehensive nursing assessments and collaborates with patients, families, physicians, and the IDG to develop, implement, and evaluate individualized plans of care.
• Maintains timely documentation and coordinates care with resources, especially in complex or high-risk situations.
• Provides education and emotional support to patients and caregivers; participates in on-call rotations, admissions, and care transitions.
• If Care Manager, coordinates all aspects of patient care including scheduling, medication management, plan of care oversight, and IDG meetings.
• If On-Call, responds to triage calls, performs visits and assessments, coordinates care with the Administrator On Call, and may attend IDG meetings.
