Job summary

Three Oaks Hospice is seeking an On-Call Registered Nurse (RN) for weekend coverage to administer skilled nursing care at patients’ homes and coordinate with the care team. The role emphasizes compassionate patient care, strong clinical skills, and effective collaboration with families and referral sources.

The RN must hold an active state nursing license, CPR certification, and have hospice/home health experience or a readiness to grow in this setting.

Qualifications

• Associate or bachelor’s degree in Nursing from an accredited program.
• Current RN license in the state and valid driver’s license.
• Two years’ RN experience in a clinical care setting; hospice or home health experience preferred.
• Current CPR certification.
• Proficient with computers; Microsoft Office experience preferred.
• Strong interpersonal skills and ability to provide effective support to the patient and family.
• Demonstrate skill in nursing practice and ability to cope with family emotional stress and diverse lifestyles.

Responsibilities

• Administer skilled nursing care for clients in their place of residence.
• Coordinate care with the interdisciplinary team, patient/family and referring agency.
